Start with a conversation.
Maybe you are writing for yourself. Maybe for a parent who has been meaning to see someone for years, or for everyone who gathers around your table. Whoever this is for, it begins the same way: a short note, read by a physician, answered with care.
The form · four fields, nothing clinical
Write to us.
A name, a way to reach you, and who this is for. That is all this page asks. Everything else (history, reports, worries) belongs in the room, with the physician.
What happens next
Read by a person.
Answered by a physician.
A physician, not a scheduler, not a bot.
Every inquiry is read by a physician, and the proposed model holds the practice to a guaranteed response window: a reply within two business days. A family that writes to us should never have to write twice.
Nothing clinical. That belongs in the room.
No symptoms, no reports, no medication lists: nothing about your body belongs in a web form. Those conversations happen with the physician, privately, where they can be held properly. This form has no clinical fields, by design.
Bring them.
Write for your father who avoids doctors, your mother who puts everyone else first, or the whole household at once. The program is built for families (shared meals, shared histories) and the first conversation can include everyone.

- The proposed access commitments (guaranteed response windows, routine appointments within 48 hours, urgent same-day care) describe the practice’s proposed design, pending launch. They are the standard the practice is being built to, not an operating history.
